In Adobe Photoshop:
Step 1 - For Photoshop
users, go File>Import>PDF image. Direct it to your
expansion pdf. A small window will open where you can scroll down
through every image in the PDF. After making your selection, click
OK.

Step 2 - Click File>New
and select Width 6.99 inches and Height 10.072 inches (For A4
size) and res at 382.81, or whatever size you want (8.5"x11"
in US). You will fit 8 cards perfectly on it easily with no
fiddling about. You can simply choose A4, but make sure the res
is at 382 or you won't have a big enough page and you will crop
cards. Choose CMYK or RBG depending on your fussiness.
Now it's a simple matter of
dragging the opened image from the pdf, onto the new page. Drag
as many as you want to print and line them up 2 across and 4 down
just like the FOTR pdf. If you only want two of a card or four,
just go File>Import>PDF Image again and find another
pic you want to place on your page. You can have any combination
of images and any amount up to 8 separate images per page. Easy.

Step 3 - Make any adjustments
to bleed, etc and print your new page. You will probably need to
realign the last page of the pdf so it lines up with your new page
if you want to print the backs.

In
Microsoft Word:
Step 1 - I've found that
the easiest way of extracting the images from the pdf's if you don't
have Photoshop is by using Word. Click Select Image from
the top menu on Adobe reader.

Step 2 - Adjust the windows
so you have both Adobe Reader and Word windows open next to each
other. Click the image you want to print and drag it onto Word.
Resize it so it so it's 3.5 X 2.5 inches using the ruler in Word.
Holding Shift while grabbing a corner of the image will allow
you to resize it and keep it's dimensions intact. So all you need
to do is Shift + click corner of image and resize it to 3.5
inches.

Step 3 - Click Ctrl
+ D to duplicate it for how many cards you want (4 or 8). Do
this after you've resized the image. That way you only have
to resize each image once only. If you duplicate it before resizing
it, you'll have to resize each and every image.

Step 4 - Line them all
up and print them. You may need to adjust your print margins.

NOTE: Simply using copy/paste
doesn't seem to work. It may work for you, but when I use the copy/paste
method, the Ctrl + D then doesn't work. It's far simpler
to just drag, resize and duplicate.
